Process of Approval of a Provider
Step one:
Step two:
- Applicant has decided to continue with approval process.
- First personal interview is scheduled in the applicant’s home where a Home Safety Checklist is completed by Agency Coordinator and a plan of action is left with applicant outlining any concerns or things that need to be changed or addressed. Applicant will receive forms and a list of checks that need to be completed. Cost of these check if any is the responsibility of the applicant.

- Three personal references from non-relatives
Medical check form
- Criminal record check
- CYIM check (Child and Youth Information Module)
- Second home visit is scheduled where the Home Safety Checklist is reviewed to ensure the appropriate changes have been made. The forms and checks completed by the applicant are handed in and reviewed by Agency.
Step three:
- Upon completion and acceptance of forms and checks the agreement is signed and applicant is contracted with Beaver Family Day Home Program to provide care for children in their own home.
- Children can now be placed in the home for care and the first home visit is scheduled for within one week of children being placed in the home.
